How to Prepare

Before your first visit:

  • Complete new patient intake forms

  • Gather all recent lab results you would like to share

  • Make a list of current medications, and supplements, including brands 

Forms and Policies

New patient forms must be completed at least 3 days prior to your scheduled appointment. Dr. Veena prefers to communicate via telephone. Texts are not an acceptable means of communication. Emails are also not an adequate means of communication when it comes to your health concerns and Dr. Veena prefers to speak with patients directly. 

Cancellation Policy

Dr. Veena requires 48 hours notice for any cancellations or rescheduling. Late cancellations are subject to a fee. Appointments cancelled the day of or if patients do not show for their scheduled visit, a charge for the full visit will be applied. 

Insurance & Payment

This is an out-of-network practice and insurance is not billed directly. However, a superbill will be provided so that you can submit to your insurance for possible reimbursement. It is your responsibility to contact your insurance for out-of-network benefits.

Payment is due at the time of service. All major credit cards and HSA/FSA payments are accepted.

Labs are not always covered by insurance. Most functional medicine labs do not participate with insurance and are out of pocket fees. It is your responsibility to check your insurance for coverage on labs prior to having them done.
